Retigabine for Seizures Definition Ezogabine (United States) or retigabine (Europe) has the chemical name N-[2-amino-4-(4-fluorobenzylamino) phenyl] carbamic acid ethyl ester. It was shown that the anticonvulsive action of ezogabine is a consequence of its ability to open the KCNQ (Kv7)-type potassium channels that are composed of four subunits and are found in the membranes of neurons [2]. The importance of this type of potassium channels in the development of epilepsy was understood only after its dysfunction was linked to benign familial neonatal convulsions [3,4]. Ezogabine binds to all subtypes of the KCNQ potassium channels, including the KCNQ2-5- subtypes that are expressed in brain tissue. The especially heteromeric KCNQ2/3and KCNQ3/5 potassium channels are highly expressed in the central nervous system and more important in the control of neuronal excitability than other types because they mediate M currents [5,6]. Ezogabine is the only anticonvulsant that has a mechanism of action that involves opening potassium channels. This fact was the initial rationale for the drugââ¬â¢s development, as researchers sought to produce a new drug that would be effective in patients with drug therapy-resistant epilepsy. Ezogabine is the first antiepileptic drug with this mechanism of action and was developed to fill this gap and help with the treatment of drug-resistant patients. Finally, clinical trials documented the effectiveness of ezogabine as adjuvant therapy for the reduction of partial-onset seizure frequency, and it received marketing authorization in 2011 in both the United States and Europe [17] The properties Ezogabine is a poorly water soluble small molecule (molecular weight 303.3). ]. The standard method for identification of ezogabine is reverse phase high performance liquid chromatography (RP-HPLC) with or without mass spectrometry using electro spray ionization and quadrupole time-of-flight mass analyzer [19]. The results of ezogabine identification could be confirmed by nuclear magnetic resonance spectroscopy and infrared spectroscopy [19]. Mechanism of action The mechanism of action of ezogabine was investigated using the electrophysiological measurements of ionic currents in Chinese hamster ovary (CHO) cells (transfected with cDNA encoding humanM1 muscarinic receptors,KCNQand CD8cDNAs) and rat ganglion cells, using the amphotericin-B perforatedpatch technique [22]. The effects of ezogabine were tested on ionic currents from both heteromeric KCNQ2/3 and homomeric KCNQ1, KCNQ2, KCNQ3 and KCNQ4 potassium channels. The potassium currents were enhanced by ezogabine through all types of channels, except KCNQ1. The effects of ezogabine were also tested in another study on HEK293 cells stably transfected with KCNQ5 channels [23]; it was shown that ezogabine concentration-dependently also enhances the potassium current through this type of channel. After binding for the KCNQ channel, ezogabine decreases the membraneà potential threshold for activation of the channel (makes it more negative, i.e., shifts it toward the hyperpolarizing end) and in creases the probability of maximum opening of the KCNQ channel [22,23]. All of these effects make neurons less prone to depolarization and activation. Pharmacokinetics and metabolism The absolute bioavailability of orally-administered RTG is ~ 60% [34]. Absorption is rapid and Cmax is typically reached 30 120 min after a single oral dose [35-38]. Cmax is moderately increased when RTG is taken with a high-fat meal, while the area under the plasma concentration-time curve (AUC) remains unchanged [39]. RTG is rapidly eliminated following first-order kinetics, with a terminal half-life of approximately 7.5 h; the clearance is 0.58 0.76 là ·h-1à ·kg-1 [35-37]. Young women may reach 30% higher Cmax values than men after accounting for body weight, but weight adjusted clearance is similar [36]. RTG has a large volume of distribution (8.7 là ·kg-1) and a plasma protein binding of about 80% [35,39]. In healthy volunteers, RTG pharmacokinetics is linearly dose-proportional for doses of 50 350 à ¼g BID and not affected by multiple administrations indicating that RTG does not influence its own metabolism [35]. In epilepsy patients, RTG dose linearity was demonstrated over the therapeutic dose range of 200 400 mg t.i.d. [35,40]. RTG is not metabolized by cytochrome P450 (CYP) enzymes [41,42]. Rather, it is extensively metabolized by N-glucuronidation, leading to formation of two major N-glucuronides, and to a smaller extent by N-acetylation, leading to formation of the N-acetyl metabolite of retigabine (NAMR, formerly AWD21-360), which also undergoes glucuronidation [41,42]. In vitro assays using recombinant human UDPglucuronosyltransferases (UGTs) identified UGT1A1, 1A9, 1A4, and 1A3 as the principal enzymes catalyzing RTG glucuronidation [43,44]. In subjects with reduced UGT1A1 metabolic capacity (Gilbertââ¬â¢s syndrome), RTG pharmacokinetics is unchanged and N-acetylation partially compensates for reduced glucuronidation, in addition to compensation by other UGT1A isoforms [45]. Other UGT1 and UGT2 family members also appear to be involved, because complete loss of UGT1A metabolic capacity (Crigler-Najjar type II) did not completely abolis h RTG glucuronidation in vitro [44]. Individuals with genetic polymorphisms in the N-acetyltransferase 2 (NAT2) gene leading to a ââ¬Å"slow acetylationâ⬠phenotype had a lower total exposure to NAMR than rapid acetylators [45]. Safety and tolerability of RTG treatment is apparently not altered in subjects with Gilbertââ¬â¢s syndrome or slow acetylation polymorphisms [45]. RTG and its metabolites are primarily excreted by the kidney [42,45]. A 30% slower RTG clearance in elderly subjects vs. young subjects is consistent with the known decline of renal function with age [36]. Systemic RTG clearance can be reduced by 50% in patients with moderate/severe impairment of liver function (Child-Pugh score â⬠¡ 7) or kidney function (creatinine clearance Safety and tolerability Three Phase III clinical trials were pivotal to the eventual approval of ezogabine for use to treat seizures. The first was a study by Porter et al. during which placebo or ezogabineà at daily doses of 600, 900, or 1200 mg was administered [9]. RESTORE 1 compared placebo to ezogabine 1200 mg/day, and RESTORE 2 compared subjects taking 600 or 900 mgà of ezogabine daily with those taking placebo [4,10]. As would be expected and consistent with other antiepileptic agents, the most commonly observed adverse effects seen with ezogabine use are CNS-related. At least 10% of study participants in Phase III clinical trials experienced somnolence, fatigue, confusion, dizziness, tremor, abnormal thinking, vertigo,à speech disorders, or amnesia [4,9,10]. Most of these adverse effects appear to be related to dose, and are typically reported to be mild or moderate in nature. As in the Phase III trials, the majority (62%) of adverse events were CNS-related. In addition, several ezogabine user s experienced neuropsychiatric events including hallucinations (2%), confusion (9%), and psychosis (1%) [12]. The majority of these episodes occurred within the first two months of drug exposure in individuals with no documented history of a psychiatric disorder.

Reflection on Patient with Acute Asthma Management Acute asthma, for a human body is really harmful. Being from a medical background, I understand the seriousness of the issue and regarding the same I want to add on to my strengths learning better to handle any unwanted situation like that overcoming my weaknesses. Here, I will discuss all the related important things in understanding the asthma problem and handling a patient quoting an example. Describe the event or area of learning / knowledge that you identified as part of your clinical scenario Example: A 45 year old female patient with asthma is to be transferred to FMC. She was initially admitted to a local regional hospital at room no. 0139 with severe asthma issue. As a first step to handle the situation she was being settled with salbutamol and amiophylline. She was then reviewed in the morning time and was suspected with chest infection and congestion. The team of the doctors discussed the issue internally and finally decided to transfer the patient to FMC for further review. Now what are the challenges and scope of learning areas I found in the case are listed below but for that I want to start from the introduction:- Acute asthma management is based on some primary steps that include assessing the severity of the patient whether it is mild, moderate, and severe or life threatening. All these will be observed during the startup of the bronchodilator treatment immediately after the patient is being admitted. Next comes the administering oxygen therapy. This is will be observed or implemented if the case is too serious and the therapy is needed to be done. If it is required then there are some things need to keep in mind that saturated oxygen titration to the target or the patient is at 92ââ¬â95% (adults) or at least 95% (children). After this is done as per the requirement, then comes the number of completing the observations and the assessments that will be solely based on clinical priorities determined by baseline severity. Asthma can be really hazardous for the patient in the longer run, they can cause chronic lung issues and respiratory problems and that is why proper observance and adminis tering is required. Further, keep a close check on systemic corticosteroids within the first hour of treatment is really necessary. Till the time acute asthma gets resolved, a regular and repeated reassessment response to the ongoing treatment is essential. If required the whole process should either be continued or something more should be added to the previous style of treatment. Also, this whole should be repeated in cycle till the patient is transferred or referred to some another hospital or ICU for further observance. Apart from all these, observing the patient for at least 1 hour a day is a must call. It was very shocking as well as sad incident for me as since my childhood I cannot see anybody in pain or in any kind of unwanted situation. To me serving mankind is the ultimate goal for my life. If by chance I could help out anybody during any bad or harmful situation through my efforts, I will be the happiest person on this earth and I mean it. And that is the reason why I chose this medical field at the first place despite of the initial oppose from my family on my decision to get into the medical field as they wanted me to be an architect. I believe that you get a chance to be born in the human race just for once, so how will it matter if I dont become the reason for someones happiness. Also, I took this situation really seriously as I wanted to deal with this kind of situation if it occurs further in my family or my neighborhood. If talking medically, at the first place as per my understanding and learning I thought of giving her salbutamol along with atrovent fused by a nebulizer as a first aid to the issue but then suddenly I planned to delay the same as I came to know that the patient has already taken her daily dose of salbutamol puffer. After having a look on her medical history, I gathered the information that the lady has some anxiety disorder and borderline disorder too. Why I am mentioning these issues here is because of the reason that just due to these disorders, a feeling of shortness of breath can be occurred and at that time I should not be or somebody else for that matter should not be panicked. Apart from these observations, her blood pressure level and heart rate were pretty normal and with these now a proper treatment plan can be taken. Also, for some further added information to the case history, while attending the patient I have observed that in the recent past she has been already admitt ed to ICU for 3 times and this should be noted down during the treatment plan. During those admissions, she complained of anxiety issues and also that she hates going to the city hospital because of their ICU referral problems. That means she totally hates being admitted in ICU as in ICU the treatment is really complex and hard to adapt. What I did well: The Primary survey, observing her social and recent medical history, collecting and placing all the facts related to her health in proper order for further planning, attaching the cardiac monitor with the patients body and then closely keep a check on the whether the assessment of heart rhythm respiratory was a thorough one or not What I could have done better: I should have given her salbutamol and atrovent nebulizer in the beginning itself, making her feel relax and comfortable by giving her instructions for proper postures, I should have also given more detailed info in handover ISBAR, the call for the second crew support was a big mistake as I could have called the local staff of the hospital itself. I will definitely make a note of these mistakes and will keep in mind for future references Also, I could have done a better research on asthmatic patients, the symptoms and the required medical approaches especially for the ones suffering from mental illnesses such as anxiety disorder. Delay in the treatment action plan and decision making was one thing needed to be checked. Apart from these mistakes, one major thing I have learnt is expiratory wheezing can indicate asthma, so I will definitely keep that in mind I take all challenges and the situations coming in my path of learning as a positive one, nothing in this world is negative so the positive thing happened with me is that I was finally able to communicate with both partner and patient really well and I am happy about my competence. Opting and deciding the correct clinical approach is another reason for my happiness on my list. I have assessed the vital symptoms and signs of the patient and also have completely referred and observed her past medical history. Not only that during the first step of the treatment, I have linked my approach and planned the treatment as per the history. If I talk about the difficulties and the barriers that I faced during the situation, then I would like to mention this that English is not my first language and during the treatment procedure I find it really hard to convey my intentions to the patient. Also, I was not able to understand her problems so I need really hard to work on that. Apart from that I believe that learning is a 24X7 process and I will definitely focus on polishing my skills further. The right to own a gun Some states in America do not consider the right-to-carry law. In 1986, only nine states in America had the right to carry a gun law. But by 1998, 31 states had this law, and more than half of Americaââ¬â¢s population live in them. Nowadays, nearly all Americans own a gun. 80% men own a gun and 20% women. About 17 million American women own a gun. Different states in America have different gun control law. In New York, there is a juvenile possession law, a permit to purchase a gun, registration, licensing of owners and a permit to carry a gun are all required. This is quite strict compared to Texas. In Texas, there is a child access prevention law but no other controls are required. There are many gun rights interest groups in the USA. One of them is the Rocky mountain gun owners. They argue that the freedoms of US people have been eroded too far already. They succeeded in blocking tighter gun control laws. Most gun control interest groups argue that a gun is the only commercially available product solely designed to kill. They believe there should be a ââ¬Ëcooling offââ¬â¢ period before people are allowed to purchase a gun from the time of application for a license. Should Children be allowed guns? In the USA, more than 135,000 guns are brought to school by pupils, some children as young as nine years old. But this is not a good idea because some children use it seriously and just shoot their friends because they are annoyed at them. Many of these teenagers are in gangs and say that they need guns for their protection. But is this true? Every 2 hours an American child gets shot, and sometimes it will be by another child his/her age.

Lebanon Lebanon, a nation that once proudly called itself the Switzerland of the Middle East, is today a country in name only. Its government controls little more than half of the nation's capital, Beirut. Its once-vibrant economy is a shambles. And its society is fragmented - so fragmented, some believe, that it may be impossible to re-create a unified state responsive to the needs of all its varied peoples. Lebanon lies on the eastern shore of the Mediterranea n Sea, in that part of southwestern Asia known as the Middle East. Because of its location - at the crossroads of Asia, Europe, and Africa - Lebanon has been the center of commerce and trade for thousands of years. It has also been on the route of numerous conquering armies. With an area of 4,015 square miles, Lebanon is one of the smallest countries in the Middle East. It is smaller than every state in the United States except Delaware, Rhode Island, and Connecticut. Lebanon is sandwiched between Syria in the north and east and Israel in the south. The maximum distance from the nation's northern border to the southern one is only 130 miles. And the maximum distance from the Mediterranean Sea to the Lebanon-Syria border is 50 miles. In the south, along the border with Israel, Lebanon's eastern border is only 20 miles from the sea. Although a tiny land, Lebanon boasts a great diversity in its landscape which makes it one of the most picturesque countries in the world. The coast line is br oken by many bays and inlets of varying size. At some points, the mountains wade silently right into the sea - then climb suddenly tier on tier away from the Mediterranean to the sky. Because of the limitation of flat agricultural land, all but the steepest hillsides have been patiently and neatly terraced and planted with garlands of twisted grapevines. The mountains lend a great variety of hues - pale pink, rosy red, forest green or deep purple - to the landscape. Depending on the time of day, they never appear the same twice, and from time to time whipped white clouds hide all except their snow-capped peaks. Even on the darkest night, the lights of the villages perched on the mountains shine in small clusters as a reminder of their presence. On c loser view, the mountains become a jumble of giant gorges, many of them over a thousand feet deep, with rocky cliffs, steep ravines and awesome valleys. These unassa... ...anon. This system worked well enough for fifteen years. From 1943 until 1958 the nation's economy boomed and Beirut was transformed into the showcase city of the Mediterranean. The government seemed stable enough, but th ere were problems boiling beneath the surface and in the mid-1950s the system began to come apart. For one thing, the Moslems, especially the poorer Shiites, had a substantially higher birthrate than the Christians; many people believed that the Shiites had surpassed the Maronites in population. But the Christians would not allow a new census to be taken, for this would have meant a reallocation of the nation's political power, with the Moslem sects gaining at the expense of the Christians. With their hopes for political gains dampened, the Shiites became disenchanted. Why is this once prosperous nation on the verge of total collapse? There are a number of reasons, but the primary one is that the Lebanese people belong to at least fifteen differe nt religious sects and their loyalty to these sects is greater than their loyalty to a united Lebanon. Had the people's sense of nationhood been stronger, they would not have suffered the destruction of the past decade.

The Western concept of value is adopted from study of knowledge, in particular philosophy and social sciences, Islam puts the revelation as a primary source of values which make education in Islam means the value of education itself. Firstly, The difference in worldview in conceptualizing human, knowledge, religion and God, have made Islam and Western education fundamentally different. Western secular education that is valuefree results in the disattachment of knowledge and science from ethics, morality and spirituality. And as such the corruption of knowledge due to the Western materialistic educational concept impacts the Islamic world which has long adopted such concepts due to western colonialization. Secondly, The Western concept of value is adopted from study of knowledge, in particular philosophy and social sciences, which then was developed in the world of education with all its relative variants. Nevertheless it is a whole different issue in Islamic perspective. The virtue of value in Islam means a belief of choice in behavior and actions under normative guidance prescribed by the revelation (wahy) of Allah and the sayings of His Messenger. In addition, education emerged as a response to moral and existential crisis caused by the Western worldview which is value-free. Value in Western conception is relative, created by human ratio and neglects the revelation. Values and morals evolve and change from time to time as a response to the social and political situation in Western civilization. Islamic conceptions of value and moral are different from Western conceptions. The value and morality in Islam comes from the revelation which was revealed to Prophet Muhammad and written in the Qurââ¬â¢an. Education in Islam is not value free and could not be separated by the metaphysical elements, but includes both the physical and metaphysical reality, and contains the principles of Islam which is useful as a guide for man to find true happiness in the world and the Hereafter. In conclusion, Value education developed by the West, should not be accepted by Muslims without adequate criticism, because there are some fundamental differences between the Western worldview and the Islamic worldview, especially in the concept of truth and knowledge. 3. Secularization and modernization of education give a huge impact on the lifestyles of Muslims. From your perspective, discuss how the concept of education can be Islamized in the Muslim majority countries? Firstly, Education is an important factor in the development of any society. The level of development of any nation is usually determined by its level of education. Therefore, Muslims should coordinate their efforts and establish their priorities, where education comes at the top. The more Muslims give to the cause of education and development at this stage to create sound Muslim mentality and psychology, the more Muslims acquire higher capabilities in all other fields. Besides, education can be seen from three aspects: a) Individual: Human is seen to have a bunch of potentials and abilities that are unknown either to the individual themselves or to others in general. Therefore, through the education process, all these potentials and abilities will be discovered and developed to benefit the individuals as well as the societies. b) Society: Education is seen to play its role as an instrument to internalize and transmit the culture from one generation to another. c) Interaction between individual and society: Education as a process of transaction between the individual and the society that is the development of potentials of the individuals will contribute to the betterment and improvement of the societies. Finally, Islamization of education, is the Islamization of the Muslim child and the coming Muslim generation. It is the process of liberating them from a culture of fear and superstition to recapture and reestablish the Muslim spirit. In short, it should be noted that the Islamization of education or the reformation of Islamic principles in education must be pursued rigorously, and the Islamic notion that man is Allahââ¬â¢s vicegerent on this earth and that he should carry on his shoulders certain duties and responsibilities should also be emphasized. That being said, the whole Muslim community should support the move of the Islamization of education. 4. ââ¬Å"Studentsââ¬â¢ academic achievement is the yardstick of the school leadershipââ¬â¢s successesâ⬠. Discuss whether you agree with this statement or not. Answer: Effective education leadership makes a difference in improving learning. Thereââ¬â¢s nothing new or especially controversial about that idea. Whatââ¬â¢s far less clear, even after several decades of school renewal efforts, is just how leadership matters, how important those effects are in promoting the learning of all children, and what the essential ingredients of successful leadership are. First of all, All current school reform efforts aim to improve teaching and learning. But there are huge differences in how they go about it. Some reforms, for example, attempt to improve all schools in a district, state or country at the same time. Other reforms attempt to influence the overall approach to teaching and learning within a school, but do so one school at a time. Still others, focused on innovative curricula (in science and mathematics, for example), typically address one part of a schoolââ¬â¢s program and aim for widespread implementation, while innovative approaches to instruction, such as cooperative learning, hope to change teachersââ¬â¢ practices one teacher at a time. Secondly, There seems little doubt that both district and school leadership provides a critical bridge between most educational-reform initiatives, and having those reforms make a genuine difference for all students. Such leadership comes from many sources, not just superintendents and principals. But those in formal positions of authority in school systems are likely still the most influential. Efforts to improve their recruitment, training, evaluation and ongoing development should be considered highly cost-effective approaches to successful school improvement. Finally, Our purpose was to summarize the starting points for a major new effort to better understand the links between leadership and student learning. There seems little doubt that both district and school leadership provides a critical bridge between most educational reform initiatives and their consequences for students.

Mostly people avoid speaking to unknown person. Internet has proved very useful for such kind of people, because they talk with people without showing their age, looks and class. People find many friends through the internet. Many public sites provide this facility. People can gain many friends through these sites. Many young people are earning money with the help of internet by web designing, game construction, working as a freelance writer. It means internet provides many opportunities for the people. Internet has shrunk the world. Today each country is connected with another country with the help of internet. People of one country or society are becoming familiar with the culture of another country or society. With the help of internet the people who have knowledge got their position in the society. At present online shopping has become very popular. People are buying products from their home with the help of the internet. People buy products at low cost from the internet as compare to market because companies can save money of shops and workers in online shopping. Because of this interest of the people in sex is increasing day by day. As a result, sex-related crimes are increasing continuously. Because of the internet a lot of jobs have been lost, because a small number of people are required to run a site. Because of the online shopping people do not leave their home. This is a reason behind the many diseases such as obesity. Online shopping is not helpful for the local economy because most of these online shopping sites are international. Internet is not always reliable, because there is a risk of breaking the privacy. There is everything is in the form of binary information. On internet we are dependent on others for our privacy. We can not surely protect our information. With the help of data mining our information can be theft. Copyright problem has become a great problem now days. Identity of a person can be theft with the help of internet. There are many other bad effects of the internet such as meeting with the people whom you met online, giving personal information to a complete stranger, giving wrong information. A person can be easily cheated by the internet. Many terrorists are using internet technology for communication by using a wrong address. Child pornography, credit card frauds, espionage, online stalking, kidnappings are some other bad effects of the internet on the society. Now it can be said that it depends on us how internet affects society. Internet can be proved very useful if we use it in a positive direction. Many laws are made for preventing internet crimes, but these crimes are still there. There is a need of enforcement of these laws. Internet can be very useful for the society. It is the best medium of communication and it can be said a good platform for expressing your ideas. There are a lot of changes in the society because of the internet. Internet enhanced the status of the living. Today information and education about the safe use of computer has become very important.
